COMMUNITY OUTREACH
Our cost-free Community Outreach program links Hamilton
County residents fifty years of age or
older with needed
resources and provides hands-on assistance in obtaining services
provided by
PrimeLife
Enrichment or other agencies and/or individuals. This service
involves an extensive initial
assessment to
evaluate specific needs and a continuing follow-up to ensure that
these needs are being
effectively
met.
When necessary, our full-time Community Outreach
Coordinator is able to respond to emergency
situations such
as:

Pending evictions
§
Abuse
§
Unsafe living conditions
§
Help with utilities
§
Need for medical attention
A facet of this program is
SHIP which can help answer
your questions about Medicare, Medicare supplement (also called Medigap) insurance, Medicare Managed care plans, Medicaid, long term
care insurance and the Indiana Long Term Care Program.
PrimeLife Enrichment hosts a Care-Giver support
group for Alzheimer's and other related diseases. Click

"Day of Caring," a yearly event, is one of the
many volunteer projects coordinated through this program. On
this day, hundreds of volunteers help seniors with a variety of
projects such as house painting and repair, the building of
wheelchair ramps, window washing and yard work.

INFORMATION AND
REFERRAL
This program utilizes the new Central Indiana Human
Services computer database to answer questions and refer callers to
appropriate resources. It is the "gateway" to PrimeLife
Enrichments other programs and to services provide by other
organizations. We encourage seniors and their families to

HOMEMAKING
PrimeLife Enrichment provides assistance for those
fifty years of age or older who need help with:

Housecleaning
§
Laundry
§
Light meal preparation
§
Shopping and errands
§
Miscellaneous household tasks
The cost for a homemaker with a two hour minimum per
visit is based on income using a sliding scale fee of $13.50 to
$18.00 per hour. It is possible to receive these services
without charge or at a reduced fee if physical and/or financial
requirements can be met and if sufficient funding is available.

RESPITE AND ATTENDANT
CARE
We provide relief to the full time caregiver and
non-medical assistance to those fifty years of age or older
with moderate disabilities who live alone but require some help with
the activities of daily living. Experienced, compassionate
aides will travel to your home and stay for as little as two hours
per day. A plan of care will be developed to accommodate
individual needs and preferences.
Every effort will be made to honor your wishes and all
assistance will be given in an atmosphere of dignity and
friendship. The cost for this service with a two hour per
visit minimum is based on income using a sliding scale fee of $13.50
to $18.00 per hour. It is possible to receive these services
without charge or at a reduced fee if physical and/or financial
requirements can be met and if sufficient funding is

ASSISTANCE TO THE
HOMEBOUND
The program links trained volunteers,
known as "Friendly Visitors," with frail and/or socially isolated
persons fifty years of age or older. In addition to
friendship, each volunteer provides a variety of services that
greatly enhance the quality of their friend's life. The
relationship is unique and as rewarding for the volunteer as it is
for the senior. Many of our volunteers are active retirees who
want to continue to make a meaningful contribution to their
community. Others are young people who enjoy and benefit from
the contact with their elders who often become surrogate
grandparents. Whatever the age, a loving friendship is always

TRANSPORTATION
Because we believe it is particularly important to help
seniors stay active and connected to their community we offer
door-to-door, non-emergency transportation to any Hamilton County
resident fifty years of age or older. Our caring staff of
drivers will pick up at your door and take you where ever you need
to go.
Our transportation service can
be used for:
For the physically disabled, we offer the use of our
wheelchair lift van.
To schedule transportation, please call Sherri at
317-815-7005 at least 48 hours in advance. Although the
demand for this service is great , we will do our best to
accommodate all requests. If we cannot take you at the time of
your particular request, we will work with you to find a mutually
convenient time. Transportation can be scheduled up to one
month in advance.
Time of service is 8:00 a.m. to 4:30 p.m., Monday through
Friday.
Service Charge: For Hamilton County residents
fifty years of age or older living independently,
transportation within Hamilton County is on a donation basis.
There is a suggested donation of $12.00 per round trip for all in county trips.
Individual donations and charitable contributions make this service
possible. In the event that a family member of loved-one may
benefit from the Transportation Program, please consider a
tax-deductible donation to the agency to help offset the cost of
this life-line program. There is a modest fee charged for out-of-county
trips.
